Q: Are the plants included?

Yes, it comes exactly like you see it! They totally look real & are a perfect little gift individually! My office doesn’t allow live plants in dirt l, so this is a great alternative. In addition my cat at home likes to eat real plants and flowers, therefore I can’t have them in my house.

Q: Is it possible to remove the faux dirt, rocks and sand and put them into another pot? I have 2 I'd like to use

No, I don't think so. The "dirt and rocks' aren't loose. They are kind of a conglomeration inside the little vase it comes in.
