This worked exactly as expected
I ordered the “used - like new” for about half the price of the sealed box. It was clean and in perfect condition. Appears it was a return where user bought it, opened it, and returned it. Open box came with sealed hardware as well as complete installation instructions. My house has 10” rough-ins. Kind of ridiculous for a house built in the 80’s but oh well. The builders actually dug a 1/2” indent in the drywall and painted it so the old toilet tank lid would fit on top of the tank. After installing new plywood (old toilet leaked) and the flooring I planned to change the set up to 12” rough-in but soon realized that would involve a lot of changes in waste pipes.I was a little apprehensive about buying this because of some of the reviews but I couldn’t return the toilet because it was too late.I have never installed a toilet before, let alone one as complicated as this model. However the instructions that came with this adapter were easier to understand than those that came with the toilet.Everything is working perfectly, highly recommend. One thing I would complain about us that I wish Koehler would sell the toilet itself without this part. Then they could sell a 10”, 12”, or 14” offset separately. I have a 12” offset that I can’t use and most likely will have to throw away since 12” ones come with all the toilets. I can’t imagine I can even give it away.

The solution
If your toilet flange bolts are ten inches from the wall but you choose to have a toilet that's made for flange bolts twelve inches from the wall...this product can make it happen. Not difficult to install if you're a handyman with the correct tools.

Works well with Kohler skirted toilets that have a 12" skirt trap already included
This flange is intended to be used with an existing Kohler skirted toilet using that uses the attachment trap system. I've seen other reviews complaining about missing bolts. Those bolts come with the toilet you bought but are unable to use the include 12 inch trap because of clearance issues. I purchased this because my supply line was directly behind my toilet and utilizing the 12 inch trap would have resulted in the shut-off being directly against the back of the skirt and leaving me with no good way to turn on and off the water supply. Using this adapter moved the toilet far enough away from the wall to allow for clearance of the supply line and shut-off.Note, this isn't just a bolt down using the toilet closet flange bolts type of system. You will need to lag it to the floor and if installing on tile and or tile over concrete, break out the hammer drill to get through the hard material in order to mount it.I installed on Jan 12, 2021 and have had zero issues with the toilet, the flange and zero leaks from this set-up (as of July 7, 2022).

Saved my bacon with a 10" rough-in
I'm a normal homeowner with limited/no knowledge of the ins and outs of plumbing/toilets etc. We bought a Kohler Gleam because happy wife, happy life and they're nice! This thing let me install the toilet she wanted with my existing incompatible 10" rough-in (this is the distance from the back wall to the center of the floor opening/bolts.Didn't know we had a 10" in until too late.Lessons learned:1)the bolts for the toilet mounting hardware screw into the front holes. Mount them prior to securing to the floor. There's a little slot the brackets slide nicely into. You'll place the screw in the smaller/ side hole of the bracket. The rear holes are for securing to the floor only. This differs from the standard 12" instructions. Wasted a lot of time with the brackets mounted to floor attachment bolts, trust me it won't work lol.2) buy some 1in bolts (3/8 size) for mounting the brackets to this thing. The included ones are too long.

Works great for a 10-inch rough-in
I purchased a Kohler toilet at a big box home center. It came with a 12-inch version of this plastic adapter. By using this 10-inch adapter, the toilet fit my plumbing, saving me thousands not having to jack hammer concrete to change the plumbing rough-in. Please note that these adapters use two additional holes in the front for attaching to the floor. I drilled into the concrete slab and used epoxy and stainless steel threaded rod for my installation. These extra mounting points make the toilet rock solid. You still need a wax ring under these plastic adapters, but it is super easy to install a wax ring without making a mistake. The toilet can also be removed and replaced without replacing the wax ring each time.

I received the 10 inch adapter. It doesn't look like this photo as it has additional 2 holes and base is shaped different than pic. It was very difficult to install.. first the supplied anchors required 1/2 inch hole and drilling this through ceramic tile was almost impossible. The supplied anchors metal toggle had no screw hole and bolts were not long enough ( needed 5 inch not 4 inch) so had to go to hardware store. I cannot reccomend this difficult to install adapter..

Instructions need clarification
First of all if you are a DYI er make sure you know what the rough in dimensions are. We found what we wanted and just thought a toilet is a toilet. Wrong my rough in was 10 inch. And we have a 12 inch rough in toilet. I needed to purchase this 10 inch rough in trap. It is just like the 12 just smaller. The instructions did not say that the hold down cams needed to be moved to the front holes. After figuring that out I went to install the bowl. It fit as designed and now gave me the proper clearance behind the wall. Now came to install the cam bolts that secure the toilet to the floor. The bolts did not have enough threads the secure the toilet. It moved from side to side and it also rocked. Not what you want in a toilet. After calling Kohler several times and waiting out the weekend, I was able to speak to a live person and they said that I would need the 3.75 inch bolts to secure my toilet to the floor. This has been one of the longest installs I have ever completed as a home owner. Too bad I had to spend another $89 for the smaller pee trap thing.
